Vårdcentralen Vilan has approximately 6900 registered patients and employees from various professional categories. We offer assessment, treatment, follow-up, and prevention at the primary care level. We have a medical clinic, district nurse clinic with, for example, diabetes care, asthma/COPD care, elderly care clinic, and a child health clinic (BVC). Our laboratory staff includes nursing assistants who handle both internal and external sample collection. Additionally, we provide advice and support to nursing homes and municipal home care.

At the health center, we also have access to well-developed rehabilitation consisting of doctors, physiotherapists, social workers, and occupational therapists aimed at optimizing the patient's rehabilitation process. The coordinator of our rehab team is our rehab coordinator, who works to guide the patient back to working life in a safe and secure manner. The work can take place in various ways, including individual and group work with patients, as well as collaboration with other professionals at the health center, employers, and the Social Insurance Agency.

JOB RESPONSIBILITIES

We now have the opportunity to welcome a social worker, preferably with CBT competence, to our wonderful team at Vårdcentralen Vilan!

Your task will be to assess and meet our patients' needs for psychosocial support through supportive and/or therapeutic conversations. You will evaluate the individual's psychosocial ill health from a holistic perspective and treat patients with various mental and social health conditions using evidence-based treatment methods. The work takes place both individually and in groups. Furthermore, it includes being active and contributing to the development of the organization through collaboration with various healthcare providers, both internally and externally, as well as with other important parties for the patient.

Within the framework of the position, there is an opportunity to develop new methods for addressing patients' needs for psychosocial support. You will also have the opportunity for personal development through regular group supervision and to join the network for professionals working with mental health issues in Kristianstad. As we are simultaneously in the initial stages of starting a Care Choice Psychotherapy program based at Vårdcentralen Vilan, we see great opportunities for the development of a collegial network even in everyday life.

QUALIFICATIONS

You who are applying are a trained social worker or have another relevant education, preferably with experience in a similar role within primary care. Furthermore, you have good language skills in Swedish, both spoken and written, as well as a valid B driver's license. Training in CBT is an advantage, and it is strongly meritorious for the position if you are a licensed health and medical care social worker. There is also the possibility of taking over the role of rehab coordinator, so we see it as positive if you have an interest and previous experience in this. To facilitate care and free up time for personal meetings, we use IT support as much as possible. Therefore, it is an advantage if you are experienced in working with various operational systems, especially in the PMO and 1177 journal systems.

It is important that you are communicative and can both collaborate and work independently. You have a good empathetic ability, a good attitude towards patients as well as your colleagues, and you see a professional approach as a given.
