About the job

Continuum Care and Nursing is a private healthcare provider within home care and home health care in Västerås.

Working in home health care means helping and supporting people with health and medical care services in their ordinary homes.

In Home Health Care, you work both independently and in teams. The tasks include teamwork with other nurses, occupational therapists, physiotherapists, and home care staff. As a nurse in home health care, you work preventively to promote the patient's health and well-being and are responsible for medical assessments, documentation, and registration in various quality systems.

Your work with us involves both direct and telephone contact with patients, relatives, and colleagues in home care, as well as documentation in our patient journal system. You will have close cooperation with home care, and most of your working time will be based at the same office as the home care group.

Some of your tasks include sampling, injections, dressings, and assessments of the patient's condition. You will also take care of patients with end-of-life care. You plan and follow up on nursing interventions. Documentation is done in Cosmic, and planning is done in Epsilon.

In your daily work, you guide and delegate as nursing staff. As a nurse, you have a significant responsibility to contribute to the continuous development and improvement of the operations.

You will use Continuum's own cars during work and will be based in our premises in Västerås.

We are looking for you who

We are looking for a licensed nurse. A driver's license and driving experience are required for the position.

As a person, you are empathetic and have an easy time meeting people who, for various reasons, need our services. It is also important that you are service-oriented and have a strong desire to contribute to the best possible care for our patients. You need to be comfortable working independently.

It is advantageous if you have previous experience from similar work and have further education as a district nurse or a specialist education in elderly care (geriatrics). We prefer that you have experience with documentation in Cosmic and are familiar with using Pascal and Senior Alert.

Qualifications/Education

*Licensed Nurse

*Good knowledge of Swedish in both spoken and written form

*We prefer that you can speak other languages such as Turkish, Arabic, etc.

*We also prefer that you have good computer skills
