Home Care Nordwest is looking for a nurse or district nurse who wants to be challenged as a care manager within municipal home care. We warmly welcome your application!

About the Position

We are looking for someone who is passionate about providing good and safe care to patients. You have a person-centered approach and an interest in building good relationships with people around you.

The positions are based in Norrby Senior, and you will work in our teams in the northern area around Svampen. This autumn, we will move to new, fresh premises and instead be based in the newly built Skebäck next to the City Park - we look forward to it!

Examples of Job Responsibilities:

As a nurse/district nurse:

• you work according to the nursing process with medical care for patients, independent planning, implementation of measures, follow-up/evaluation of measures
• you provide advice and guidance, perform examinations and treatments based on independent assessments
• you work with preventive health care
• you manage medication and delegate tasks to nursing staff
• document in various quality registers and operational systems

As a district nurse:

• you have an extra responsibility to convey structured working methods to your colleagues
• you have prescribing rights and use your knowledge of the properties of medications, prescriptions, and their side effects
• based on in-depth knowledge, make health economic choices for investigation and treatment
• you guide colleagues in areas with training needs and are a driving force towards Close Care

About the Workplace

Home Care Nordwest consists of three units and we have a large geographical catchment area from Södra Grev Rosengatan and north. We have offices at Kornellen, Boglundsängen, and Norrby Senior. With us, we work in teams consisting of nursing assistants, occupational therapists, nurses, and district nurses. We also have close cooperation with home care services and Olaus Petri health center.

The home care is organized within the Social Administration's area of Care and Support. The operation includes care and support, preventive activities, authority exercise in the area, as well as housing placement and housing adaptation. Within the area of Care and Support, we work to support people in living a healthy life and being able to continue participating in community life on their terms.

Qualifications

You take responsibility for your tasks in an independent and structured manner and have the ability to make your own decisions. You are attentive and accommodating in your approach, both towards colleagues and patients, and you see it as important to build trusting relationships. Furthermore, you enjoy coordinating and being solution-oriented in what you face.

Additionally, you are clear in your communication and master the Swedish language in such a way that you can communicate with patients, relatives, and colleagues in both speech and writing. Since the work involves digital documentation in operational systems, good computer skills are required.

We place great emphasis on personal suitability.

Requirements:

• licensed nurse
• driver's license B for manual transmission

Meritorious:

• district nurse or other relevant specialist education (geriatrics/care of the elderly and psychiatry)
• experience in municipal home care
• experience with operational systems, such as TES, Treserva, MCSS, Pascal, Lifecare

Working hours: day, evening, and weekend (38.25 hours/week)
