Sabbatsbergsgeriatriken is a pleasant clinic that provides acute geriatric care near Odenplan, in the heart of Stockholm.

This means that we work with assessment, treatment, and rehabilitation of multi-ill elderly patients in both inpatient and outpatient care. In addition to our wards, we have been pleased to expand our care with osteoporosis and cognitive clinics, as well as enhanced discharge services.

About the position

Our patients often have complex needs and multiple chronic diseases, which places high demands on team collaboration. You will be responsible for assessing and training physical functions, prescribing and providing assistive devices, participating in SIP meetings and team rounds, and collaborating with other healthcare providers in the care chain. You will also be involved in training nursing staff regarding transfer techniques and early mobilization.

With us, the patient is in focus, which is why we work in teams with nurses, nursing assistants, occupational therapists, dietitians, speech therapists, social workers, and doctors.

Additionally, we have initiated the enhanced discharge process. The purpose is to offer frail elderly continued support and follow-up from the multiprofessional geriatric team shortly after discharge from inpatient care. With this, we hope to provide even safer homecomings and reduce the risk of acute readmission. We hope that you, as an applicant, are interested in being part of this!
