About the Role

In SAM Focus, we work closely with people in transition. You will become part of individuals' development journeys while contributing to a larger context within Växt-huset. Our work is based on collaboration, participation, and a shared responsibility to create sustainable results.

Here, we work long-term with the individual in focus – where we together support, coordinate, and create conditions for step-by-step development.

In Skurup, there is an ongoing project funded through the coordination association Finsam/Nils. The project is called SAM Focus, and its purpose is to coach referred individuals from the Swedish Social Insurance Agency, the Swedish Public Employment Service, the Region, or the Municipality, so that a positive step forward towards rehabilitation cooperation and self-sufficiency occurs for the individual. It also involves leading an inter-agency team with collaboration partners within Finsam/Nils. The project is part of the Växt-huset activity, which targets people who need steps towards improved mental health and towards independence and self-sufficiency.

Responsibilities

We are looking for a job consultant with experience of the target group of individuals who are far from working life due to mental health issues or physical ailments. Through a coaching approach, the individual is given an individual plan filled with activities that jointly contribute to progression.

Participants are referred from the collaboration partners in Finsam, where goals are set based on the individual's needs. As a job consultant and coordinator, you will create an initial planning and mapping together with the individual. You will then have close contact with supervisors in the activity where the activities are carried out, followed by overall follow-up with collaboration partners from Finsam/Nils.

You will, together with a colleague, act as a gateway and ensure that the right methods and approaches are applied at Växt-huset. The inter-agency team contributes to increased chances of good planning and minimizes gaps that could cause the individual to lose motivation in the initiated development work.

You will report on the project's progress and lead the follow-up part to the board of Finsam/Nils.
