Your Health Centre in Söderhamn has approximately 6,600 registered patients and offers a broad range of services including family doctor clinics, emergency care, rehabilitation, a training pool, a psychosocial team, and Child Health Services (BVC).

RESPONSIBILITIES

As a Diabetes Nurse at Söderhamn Your Health Centre, you are responsible for the daily operations of the diabetes clinic. You work independently with patient meetings while being part of a multiprofessional team, collaborating closely with doctors, rehabilitation staff, and other professions to provide patients with secure and high-quality care.

At our clinic, you will among other things:

• Be responsible for your own diabetes clinic.
• Assess, treat, and follow up with patients with diabetes at the primary care level.
• Provide advice, support, and education to patients to strengthen self-care.
• Work together with a colleague within the diabetes field.
• Collaborate closely with doctors and other professions, for example within rehabilitation.
• Contribute to the development of diabetes care and a person-centered approach.

QUALIFICATIONS

We are looking for a flexible and engaged person who thrives in close collaboration with all professions at the health centre in a varied daily life. You are interested in development and improvement work, are confident in your professional role, and have the ability to meet people in a professional and empathetic manner.

In the role of Diabetes Nurse, you must:

• Be a registered nurse or district nurse with a specialization in diabetes.
• Hold a valid Category B driver's license.

For this position, it is meritorious if you have experience working in primary care, experience in elderly team work, Lifecare, and the medical record system Cosmic.
