Working as a Social Worker and Rehabilitation Coordinator at WeMind

The social worker acts as the link between healthcare and other societal actors, contributing the psychosocial perspective within psychiatry, alongside medical and psychological expertise. Read more about the role of social worker/rehabilitation coordinator.

You will be part of a multidisciplinary team and contribute with knowledge about patient rights and opportunities for support in society. We have high ambitions for close collaboration within the clinic, as well as with our patients, care partners, user organizations, and relatives. There are great opportunities for specialization within the profession.

Main Responsibilities:

  • Assess the patient's social situation and identify needs for support and interventions.
  • Coordinate interventions and collaborate with relevant actors such as care partners, social services, the Swedish Social Insurance Agency, and the Swedish Public Employment Service.
  • Convene and lead network meetings/SIP meetings.
  • Support sick-listed patients in the rehabilitation process and their return to or entry into working life.
  • Collaborate with actors involved in the rehabilitation process.
  • Provide consultative support regarding social issues to other professions within the organization.

Your New Workplace

WeMind Liljeholmen is a general psychiatric outpatient clinic that treats all types of psychiatric conditions except for pure psychotic disorders. We are Sweden's largest psychiatric clinic and offer a broad range of treatments to our patients. You will work under leadership with close clinical anchoring, enabling fast and pragmatic decision-making paths. The DBT team at Liljeholmen has been in place for a long time and is well integrated into the clinic's way of working.

The clinic is located in Liljeholmen, approximately a 5-minute walk from the red line of the subway.

About You

  • Social work degree (Social work education).
  • Experience from a similar role within psychiatry is meritorious.
  • Knowledge of the medical record system TakeCare is meritorious.

As the hub in the network, it is important that you are flexible, professional, and have good collaboration skills. You have the ability to independently manage individual cases as well as plan and organize.

You are highly committed to your work with a high service level for patients who often have more severe mental disabilities. You contribute to a good work climate and value the social work environment and team spirit.
