As a counselor/care coordinator with us, you combine clinical, patient-oriented work with the coordination and organization of care efforts. You contribute the psychosocial perspective to assessment, treatment, and collaboration, and are an important link between the patient, guardians, the clinic's team, and external partners.

The role suits you if you want to work closely with patients and guardians, while also enjoying creating structure, keeping care processes together, and contributing to an efficient and secure patient flow.

You will, among other things, work on conducting psychosocial mapping and assessment of children, young people, and families. You will have supportive, advisory, and therapeutic conversations with patients and/or guardians, and serve as a contact person and support for patients and guardians throughout the care chain and in collaboration with schools, student health services, social services, habilitation, primary care, and other healthcare providers. You will also participate in the development of the clinic's routines, patient flows, and quality work.

Qualifications: Social worker with experience working with children, young people, and families. It is meritorious if you are a licensed healthcare counselor and/or have a basic psychotherapy education, CBT focus. You have experience of both patient-oriented work and coordination of care efforts, as well as good experience of collaborating with guardians, schools, social services, and other external actors.

Personal qualities: You are confident in your profession, structured, and have good prioritization skills. You find it easy to collaborate, build trust in meetings with children, young people, and guardians, and appreciate working in a cross-professional team. You are solution-focused, communicative, and have a genuine commitment to the mental health of children and young people. You enjoy combining clinical work with coordination and want to contribute to making healthcare better.
