WeMind Individual & Family was created with a clear vision to raise the level of care offered to society's youth. It must be a given that facilities receiving individuals with such vulnerable life situations that they require 24-hour placement have the highest density of competent treatment resources, take active responsibility for using evidence-based methods, conduct rigorous follow-ups, and simultaneously keep societal costs as low as possible.

We are now looking for a Social Worker who wants to work with us in Skrubba (Tyresö) on a full-time basis from September to early January.

Your new workplace

WeMind HVB Evia is a treatment home with high quality and competence. Young people with neuropsychiatric diagnoses, boys aged 13 to 19, live with us. We make a difference for our young people every day through our approach and qualified treatment. The facility's treatment work is based on learning psychology and behavioral analysis, and this methodology permeates both individual therapeutic interventions and the daily approach of all staff members towards the young people.

Evia has been part of WeMind – Individual and Family since 2022. WeMind has a unique focus on evidence-based care interventions and continuous evaluation of care outcomes, and through efficient, equitable, and evidence-based care, WeMind makes a real difference.

HVB Evia is located in Skrubba, Tyresö, where treatment-responsible psychologists work together with the director. The treatment work is carried out in close collaboration with the facility's treatment assistants. The facility also includes a nurse and a psychiatrist.

Working as a Case Manager Social Worker at WeMind HVB Evia

The position of Case Manager is newly established at Evia HVB, which implies both challenges and opportunities. You will have some freedom to shape the role, something we do continuously in consultation with each other.

In the role of Case Manager Social Worker, you will be responsible for our youth cases. This is done in interdisciplinary treatment teams together with a psychologist and a treatment assistant (contact person). The treatment work is based on ABA (Applied Behavior Analysis) and Positive Behavior Support (PBS). We base our work on the young person's individual goals and the social services' care plan prior to placement.

As a Case Manager Social Worker, you have special knowledge of the holistic perspective and the young person's network. You are responsible for needs assessment, planning, and evaluation of interventions during all phases of the placement. Your work is carried out in close collaboration with the young person themselves, relatives, social services, and the professional network. As a social worker, you are an active part of the ongoing environmental work, which can sometimes be both demanding and practical.

About you

  • Degree in Social Work
  • Minimum 1–3 years of experience as a Social Worker
  • Valid driver's license (Category B)
  • Extract from the criminal record for work at HVB (Residential Care Home)
  • Meritorious: Experience from positions with our collaborating partners such as social services (authority for children and youth), schools, LSS (Support and Service Act), or child psychiatry
