Welcome to Alby Health Centre, a well-functioning medium-sized health centre in a multicultural area. We are a lovely team of approximately 30 employees where closeness facilitates our collaboration. Our staff includes doctors, nurses/district nurses, nursing assistants, medical foot care providers, home care services, and our own laboratory. We work with a health-promoting, population-oriented mission to improve the health of our community.

We have approximately 70 patients registered in our home care service, and the team consists of nursing assistants, nurses, and a district nurse.

We are actively working to develop our wonderful health centre and are looking for you who are interested in driving development.

The health centre is located in Alby Centre, right next to the subway station.

We are looking for you who are a registered nurse with experience in home care.

Responsibilities: Standard duties such as assessments, admissions, collaboration with other actors, SIP (Individualised Care Plan), etc.

Qualifications: Registered Nurse. Experience in home care and the electronic health record system Take Care is desirable.

Personal qualities: You thrive working independently, are flexible, and enjoy being part of a team. You are confident in your profession. You have a desire to develop home care and drive it forward.

Employment type: Permanent position, full-time. Probationary period may apply.

For patient-facing work within the Stockholm County Council (SLSO):

• A background check is applied before the decision on employment according to SLSO's overarching guidelines for background checks upon employment.
• Employees who will have patient contact must undergo a pre-employment health examination, according to current directives, which takes place through occupational health services before employment.
• Good knowledge of the Swedish language corresponding to level C1 according to the Common European Framework of Reference for Languages (CEFR) is required, in addition to a Swedish professional license. The same requirement applies to non-licensed staff with patient contact.
