About the position

The High-Risk Clinic is part of Karolinska University Hospital in Solna and works with preoperative assessments based on Comprehensive Geriatric Assessment (CGA), an international care concept based on team-based and person-centered assessment of elderly patients with complex care needs. Our main task is to assess and optimize patients prior to major surgical procedures.

As a nurse, you work closely with doctors, assistant nurses, and other professions in a multidisciplinary team. Together, you conduct risk assessments and design individualized care plans focusing on the patient's function, resources, and risk factors.

In this role, you will:

• Participate in multidisciplinary risk assessments and design individualized care plans.
• Conduct medical assessments and follow-ups in close collaboration with doctors, assistant nurses, and other professions.
• Work in a team-based manner based on CGA, focusing on a holistic view, function, risks, and resources of the elderly patient.
• Contribute to the development of new and innovative care models as well as research within geriatric and perioperative care.
• Provide support, information, and education to patients and their relatives.
• Act as a contact nurse for patients assessed at the High-Risk Clinic.

Qualifications

Requirements:

• Swedish nursing license.
• At least five years of experience.
• Good knowledge of Swedish, both spoken and written.

Meritorious:

• Experience from outpatient or inpatient care with elderly patients.
• Specialist education in geriatrics, neurology, anesthesia/intensive care, or another related specialty.
• Competence in areas such as motivational interviewing, physical activity for the elderly, or other relevant assessment and treatment methods.
