District Nurse/Nurse to Home Healthcare

Home Healthcare
The goal of home healthcare is collaboration for good and safe care, ensuring that patients feel secure, well received, and involved in the design of the services we offer.
Home healthcare is divided into multiprofessional teams consisting of district nurses/nurses, occupational therapists, physiotherapists, and nursing assistants. Home healthcare is organized into three geographical areas: northern and central Gotland, Visby, and southern Gotland.
Medical interventions are managed by the patient's health center and, in some cases, by the responsible physician at the specialist clinic. We also work on behalf of 1177 and the palliative team.
We are now looking for two district nurses/nurses
One position for the southern home healthcare team based in Hemse and one position for the northern/central home healthcare team based in Visby or Slite.
Your Responsibilities
As a district nurse/nurse within home healthcare, you will carry out planned and sometimes urgent interventions within your geographical area of responsibility. The work is free and independent, and you plan your days in consultation with your colleagues. In addition to patient-related work, including assessments, examinations, information, and advice, you also play an important role in collaboration with occupational therapists and physiotherapists in your multiprofessional team, as well as with the base staff in home care and with physicians in primary care. The work also involves close contact with other external healthcare providers and representatives for the patient.
As a district nurse/nurse within home healthcare, you will work daytime with duty every fourth weekend. Some evening shifts may become relevant in the future.
Who are you?
We are looking for a licensed nurse with experience in the profession. A B driver's license is required.
It is an advantage if you have specialist training. You are confident in making independent decisions and assessments, prioritizing based on the situation, and are used to working closely with others.
In addition to good collaboration skills, you are relationship-oriented in your interactions with patients and colleagues, and it is important for you to be perceived as empathetic and attentive. You are a team player who considers the whole and the best interests of the workgroup.
