JOB DESCRIPTION

The Homecoming Team in Ljungby Municipality works to provide support and care during the transition from hospital or short-term care. The team is cross-professional and works closely with both health care and the regular home care services. The goal is to create safe, effective, and individualized interventions in the home from the first day after discharge.

As the assignment grows, the position will also include staffing in home care groups as well as more specialized tasks.

As a care assistant with us, you will among other things:

• Perform care and service tasks in the clients' homes.
• Collaborate with nurses, physiotherapists, occupational therapists, and regular home care staff.
• Participate in planning and follow-up of the homecoming process.
• Work with delegated health care tasks.

The position will eventually also include:

• Staffing in regular home care groups as needed.
• Assignments requiring advanced skills and delegation, for example: Track care, handling of neck braces, and other advanced care tasks with special delegation.

This means you will have a varied and developing role with both breadth and depth.

QUALIFICATIONS

We are looking for someone who:

• Is a trained care assistant (requirement).
• Has a valid driver's license (requirement).
• Has a good ability to work independently and make decisions on the spot.
• Interacts with people professionally and confidently even in changing situations.
• Thrives in a team where cooperation and flexibility are crucial.
• Is comfortable receiving and managing delegations.
• Has experience from home care or municipal health care (requirement).
