Salem Health Center has approximately 14,300 registered patients and is a stable and well-functioning operation with a well-documented good working climate.
We conduct our operations in modern and pleasant premises located in a quiet and nature-friendly area with villas, townhouses, and a small number of apartment buildings. Salem is a geographically small municipality, which means short distances and proximity to our patients.

At the health center, there is an independent and experienced nursing group responsible for their own clinics in diabetes, hypertension, asthma/COPD, and dementia. We also have a highly competent psychosocial team consisting of two social workers and a rehabilitation coordinator. In addition, we conduct primary care rehabilitation with physiotherapists, occupational therapists, chiropractors, and dietitians.
Our mission also includes a child health center (BVC) and family center in collaboration with the municipality. A midwifery clinic is located in the same building. The health center has an easily accessible location with good communications from southern Stockholm and Södertälje, close to both the E4 and commuter trains.

Job Responsibilities
You will, together with other district nurses, be responsible for home healthcare. The position includes all commonly occurring tasks within basic home healthcare and involves close collaboration with our competent nursing assistants, the municipality's home care service, and the health center's home healthcare doctors.
The work is characterized by great freedom under responsibility, where you as a nurse plan and structure your workday and your patient visits in consultation with colleagues and other team members. There may also be opportunities to work in the clinic as needed.
Qualifications
- Licensed nurse or district nurse with an interest in home healthcare.
- Driver's license B.
- Swedish language skills equivalent to C1.
- Experience from previous work as a nurse and confidence in your competence.
- Experience in primary care and the TakeCare journal system is advantageous.

Additional Information

- background checks are applied before a hiring decision according to SLSO's overarching guidelines for background checks during employment.
- employees who will have patient contact must undergo a pre-employment health examination, according to applicable directives, which is done through occupational health services before employment.
- in addition to Swedish professional certification, good knowledge of the Swedish language equivalent to level C1 according to the Common European Framework of Reference (CEFR) is required. The same requirements apply to non-licensed personnel with patient contact.
