About our open position

As a nurse with us, you will follow the patient over time and plan actions based on the patient’s entire life situation.

The work is done in close collaboration with specialized care and municipal primary care. The role of the nurse in the team involves both planned and preventive care as well as unplanned assessments and treatments at home. Home visits are conducted both independently and/or in teams. Therefore, it is important to feel secure in performing standard nursing tasks while also having a strong desire to work collaboratively. The most important function is to be the "spider in the web." You should be able to independently prioritize, treat, and follow up based on the needs and symptoms of the multi-sick elderly patient. This is done in close cooperation with the team. Telephone counseling, information, and consultation for patients/relatives and caregivers are significant and essential parts of the job.

Working hours are scheduled during the day, Monday to Friday.

Jointly improving care for elderly individuals with significant and complex care and support needs requires collaboration with primary care, municipalities, and hospitals. The local healthcare team is part of the hospital's mobile specialized care and consists of experienced nurses, social workers, dietitians, secretaries, and specialist doctors.

Your profile

We are looking for someone with a Swedish nursing license and a couple of years of professional experience who is interested in working with multi-sick elderly patients. Experience or specialized training in elderly care or as a district nurse is an advantage. We prefer that you have an understanding of the current organizational pathways within municipalities, primary care, and specialized healthcare.

You have good collaboration skills and are communicative, with a desire to build strong relationships with both patients and relatives as well as other actors around the patient. As a person, we want you to be attentive, flexible, and independent. You should be a problem solver who dares to think broadly. We place great importance on personal suitability. A driver's license is a requirement.
