The position is placed within the Health and Care Administration, which is one of eight administrations in Linköping Municipality. The Health and Care Administration is responsible for the municipality's elderly care, health care, authority enforcement, ordering services, and provides care and support in its own right. Your Responsibilities
The mission of home healthcare is to provide health and medical care at home to those in need. Our services must maintain a high quality and be designed based on each individual's unique needs, respecting integrity and self-determination. We also work health-promoting and preventively by focusing on early interventions and strengthening the individual's own ability. We collaborate with the health center and work to instruct and guide nursing staff in the care process around the patient.
Working as a nurse in home healthcare involves many varied tasks. You work in the patient's ordinary residence where you meet patients with varying care needs. You are responsible for making independent assessments as well as leading and performing medical interventions. You will, among other things, plan measures and conduct follow-ups, manage medications, infusions, and medical technical equipment, and assist in the doctor's authority enforcement. Both planned and urgent patient visits occur.
As a nurse in home healthcare, you plan and implement educational initiatives that enhance the medical competence and care quality of the staff. You actively drive educational initiatives and are responsible, in close collaboration with the operations manager, for developing a strategic development plan moving forward. In addition to your clinical expert support in daily care for nursing staff, you are responsible for delegations for both regular staff and substitutes.
Your Workplace
Home healthcare in Linköping is divided into eight offices. You work independently but are part of a knowledgeable and dedicated team of colleagues. Together, you are responsible for a specific geographical area. We are currently looking for someone to work in one of the following operations: Blandaren, Linghem, and Ljungsbro. Blandaren is responsible for the central city area while Linghem and Ljungsbro are responsible for their respective localities and surrounding areas.
You belong to the management group in the operation you work within, together with the operations manager, coordinator, specialist nursing assistants, and physiotherapists and occupational therapists. A pharmacist also works within home healthcare. We work according to a vision of multiprofessional teams where you collaborate with colleagues in other professions to provide qualitative care based on a holistic view of the patient.
Your working hours are mainly scheduled Monday to Friday from 8:00 AM to 4:30 PM. Within home healthcare, you have a rolling 6-week schedule where you work one weekday evening and one weekend. Evening and weekend work occurs in our on-call service where you meet patients across all elderly care, LSS, and social psychiatry. Who We Are Looking For
We are looking for a licensed nurse in Sweden. It is meritorious if you are a trained specialist nurse in palliative care, elderly care, or district nursing, or another direction that the employer assesses as equivalent.
You need to have previous experience working as a nurse, and it is meritorious if you have worked as a nurse within municipal health and medical care. We consider it an extra merit if you have also previously worked in home healthcare. Additionally, it is meritorious if you have worked as nursing staff within municipal care and support.
As the job involves documentation and contacts with patients and relatives, it is a requirement that you have good knowledge of Swedish, both written and spoken. You also need to have a B driver's license for manual transmission and be able to cycle.
As a person, you work well with others, listen, communicate, and resolve conflicts constructively. Furthermore, you have the ability to lead and coordinate work by creating engagement and participation. You also understand how individuals absorb knowledge and can therefore adapt how you convey information to achieve common goals.
In your work, you can plan, organize, and prioritize your work and easily adapt to changing circumstances. You make correct assessments and have the ability to weigh complex information and different types of considerations.
