We are looking for an Engaged Rehab Coordinator!

Do you want to be the link that makes a difference? As a rehab coordinator with us, you play a central role in coordinating the sick leave and rehabilitation process.

You are a knowledge bearer in insurance medicine issues and serve as support for the patient, the doctor, and other health professionals. You contribute to creating a safe, structured, and effective path back to activity and work for patients with reduced work capacity. We work in teams and are a wonderful group with a lot of openness.

Your main responsibilities

• Coordinate rehabilitation efforts and contribute to a functioning flow between the patient, care team, and external parties such as employers, the Social Insurance Agency, the municipality, and the Employment Service.
• Support for the patient and care team in the sick leave process.
• Participate in review meetings and contribute to planning for return to work.
• Contribute to the development of the unit's insurance medicine work.
• Compile and present statistics on ongoing sick leaves.

We are looking for someone who

• Has a relevant university degree in, for example, health care, behavioral science, or social work.
• Has experience in rehabilitation work and preferably in coordination.
• Is structured, communicative, and has good collaboration skills.
• Has knowledge of the social insurance system and the roles of various actors.
• Is a person who can see both the organization and the patient from a holistic perspective.
• Has good communication skills and an ability to build good relationships with both patients and colleagues.
• Is attentive to our patients' total needs and meets everyone with a professional and humble approach.
• Is stable, easily adapts to different situations, and works well in teams as well as independently.
• Enjoys taking the responsibility and showing the commitment required in a small unit.

Qualifications

Licensed nurse, physiotherapist, occupational therapist, or social worker. Completed the Karolinska Institute course "Coordination Efforts - Coordinating the Sick Leave & Rehabilitation Process (7.5 credits)" or have equivalent knowledge of insurance medicine.
