We are now looking for a nurse for a special assignment as a coordinator within ASIH Aleris North who wants to help strengthen our collaboration and role in the care chain?

About us

Aleris offers advanced home healthcare (ASIH) and specialized palliative care to patients who need hospital-level care but can be safely cared for at home. Our care is person-centered and based on collaboration, quality, and compassion.

ASIH North has units in five locations in northern Stockholm and consists of multiprofessional teams with doctors, nurses, physiotherapists, occupational therapists, dietitians, and social workers. We have extensive experience in caring for patients with complex medical care needs.

The Role

As an ASIH coordinator, you play a key role in promoting effective, solution-oriented, and person-centered collaboration with referrers such as acute hospitals, health centers, clinics, and nursing homes. You act as a hub in our organization and work both strategically and practically to ensure high quality in care transitions and referral management.

• The assignment includes:
• Daily contact with referrers through visits, digital meetings, and phone calls.
• Planning joint meetings and training sessions with referrers to strengthen our collaboration.
• Guidance in the referral process and follow-up of referral assessments and admissions.
• Collaboration with units within ASIH to follow up on routines, strategies, and quality.
• Documentation and logging of contacts as well as follow-up of referrer satisfaction.
• Participation in APT, planning days, and if necessary, also management meetings, unit manager meetings, and doctor meetings.
• Collaboration with other ASIH coordinators within Aleris.

About You

• You have previous experience of ASIH as a form of care
• You are a licensed nurse with an interest in advanced home healthcare
• You are confident, independent, and open to new challenges
• You have good collaboration skills and appreciate working in a multiprofessional team
• You are professional in your interactions and have the patient's best interests at heart. You are a positive, engaged, and communicative person who actively wants to contribute to our development.
• Confident in your professional role, easy to collaborate, and have a broad medical competence.
• A driver's license is required.
• You have good knowledge of Swedish, both spoken and written.

Employment Conditions Employment form: Permanent position as a nurse with a special assignment, starting with a 6-month probationary period. Scope: Full-time Start date: By agreement Salary: Individual salary setting Collective agreement exists.
