The Mobile Team Primary Care is located in several places in Skåne and is experienced very positively by both patients and staff. In December 2023, we started the team in Kristianstad and in June 2024, we started the team in Hässleholm, covering the areas of Kristianstad, Bromölla, and Östra Göinge municipalities as well as Hässleholm, Osby, and Perstorp.

The healthcare providers involved include the Central Hospital in Kristianstad (CSK), Hässleholm Hospital, ambulances, private and public health centers, and other healthcare providers in the area, such as the health and care administration in the respective municipalities.

The Mobile Team Primary Care is a cohesive form of care where primary care is the hub in collaboration with other healthcare providers. The team offers safe, clear, and accessible care for patients who need medical care at home instead of hospital care, where the team can assess, plan, prescribe, and perform interventions. The care is specialized according to each individual patient's needs, and the goal is to offer patients qualified care both medically and psychosocially.

The team is based in each location and consists of doctors, nurses, social workers, and medical secretaries.

As a nurse with us, you will provide advanced medical care directly in the patient's home. Our goal is to assess and treat on-site, where you will play a central role in planning and coordinating the patient's daily care together with other professionals. In addition to clinical work, you are expected to actively participate in the development and change processes of the organization, where your experience and creativity will be important for building and improving our form of care.

QUALIFICATIONS

We are looking for a licensed nurse with the authority to practice within Swedish healthcare. Language skills in Swedish at least at level C1 according to the Council of Europe's scale are required. You also have previous experience working as a nurse. For this position, it is also a requirement that you have a B driver's license, as care is provided in the patient's home. We consider it an advantage if you have experience or specialized training in any of the following areas: internal medicine, infectious diseases, or primary care, as we desire a breadth of competence in the team.

The Department of Close Care and Health has existed since January 1, 2025. The department is responsible for grant-funded care activities within close care. This includes mobile teams and specialized palliative care, outpatient activities within close care, acute care within close care, 1177 by phone, and some region-wide functions. In addition, the department is responsible for the development and follow-up of the transition to close care in Region Skåne. The number of employees is expected to be around 1,000.
