JOB RESPONSIBILITIES

You will be based in Kåge and perform primary care interventions in patients' homes. Your main task is to be a fixed care contact for patients living at Hedvigsgården, but you are also expected to provide care and support to other patients in the northern area, both those living in ordinary housing and those living in nursing and care homes. This role offers great variation, flexibility, and the opportunity to face new challenges every day. Together in the northern collaboration area, we work every day to support and help each other so that patients receive the right care at the right time.

Our health and medical care unit works to provide patients aged 18 and over with quality care at the primary care level in the patient's home. As a nurse with us, you will have an independent, varied job where planning and prioritization are important parts. You will work with assessment, planning, execution, and follow-up of health and medical care interventions in the patient's home. Together with an engaged health and medical care team, you will also guide and support other professional groups to help them grow in their professional roles. The work is mobile and changing with many different contacts with patients and relatives, colleagues, and others within and outside the organization where the region is an important partner.

You play a crucial role in shaping person-centered care based on each patient's unique needs and wishes. With us, you become part of a professional and committed group. We continuously work to improve care quality and meet the changing needs of both patients and society.

QUALIFICATIONS

We are primarily looking for you who are a licensed nurse, or soon to be licensed. If you have previous experience working as a nurse, it is an advantage. A good subject competence and willingness to stay updated in your professional field are prerequisites for succeeding in the role. It is advantageous if you have completed specialist training in relevant areas.

As a person, you are independent, proactive, and responsible. The work requires that you have a careful and systematic approach, an eye for detail, and the ability to prioritize correctly. You are pedagogical in your communication, considerate in your interactions, and enjoy working closely in a team. A B driver's license is a requirement.

To succeed in the role, you need to have good communication skills in Swedish, both spoken and written. Skellefteå municipality is a Finnish and Sami administrative municipality, so it is advantageous if you can speak Finnish or Sami. If you can also use sign language, we see it as an asset in our operations.
