Do you want to become part of our team with great commitment and heart? We are looking for you who want to work within the municipal primary care with a focus on health and medical care in ordinary housing.

We are looking for a nurse for the Municipal Primary Care unit to work within the home care team, which also includes functional support. You will be part of teams with nurses, but also in a cross-professional team with occupational therapists, physiotherapists, unit managers, basic staff, and other professions surrounding the patient, with a focus on patient safety and person-centered care.

You have great opportunities to plan and structure your working day so that it works for you, your patients, and your colleagues. You will be the responsible nurse for a geographical home care area, which includes daily driving.

Responsibilities

Your main responsibilities are to coordinate and ensure that specific care interventions are planned, implemented, and evaluated, as well as to educate, supervise, and delegate to basic staff. You perform medical assessments, specific care interventions, and work in a health-promoting and preventive manner.

The work involves planning and following up on care interventions independently or together with colleagues. Systems that support us in our work include, among other things, digital signature lists and the journal system Life Care HSL.

Qualifications

We are looking for you who are a licensed nurse (Swedish license) with a commitment and interest in municipal primary care with a focus on home care. You have several years of experience working as a nurse, and it is meritorious if you have worked with similar tasks previously. We would appreciate it if you had specialist training with a focus on district nursing, elderly care, or psychiatry.

To succeed in the position, you work independently and take significant responsibility for your profession as well as for the planning and follow-up of health and medical care. You value teamwork highly and have a strong interest in working with frail elderly people, dementia, and palliative care. As a person, you are stable and flexible, have a high sense of responsibility, and have the ability to work both independently and in a team. You are driven by challenges and want to develop both within your profession and within municipal primary care. We place great emphasis on personal suitability and the ability to work with different tasks based on the needs of the operations.

Conditions

You work days, evenings, and approximately every 5th weekend, and have flexible working hours. We can offer you a wellness allowance, individually adapted introduction, a mentor, and collegial supervision.

A valid B driver's license (manual transmission) is required.

Two reference persons, at least one of whom is a manager, are to be provided during the interview. In connection with any employment, we request an extract from the Police's criminal and suspicion register. A record may affect your chances of employment.
