We are now looking for a nurse/advanced practice nurse for Hallunda Primary Care Centre for home care.

Responsibilities:

Here we work with basic home care, and healthcare interventions take place in the patients' homes.

As a nurse, you collaborate with other nurses in home care and have a large team of nursing assistants whom you support in their work with patients.

You are the team leader for your group together with your nursing colleagues who are on duty at the same time, and visits can be planned together with doctors in home care. This applies to both scheduled and unscheduled visits.

To succeed in the role of nurse with us, we believe your heart beats a little extra for home care and that you are inspired by a varied work in a central role. You possess good communication skills and are confident in your professional role. You should find it easy to collaborate with others and have broad medical competence. You thrive in a changing environment and need to be structured and have a solution-focused perspective.

All tasks at a primary care centre may occur, but your main focus will be on Home Care.

There is an opportunity to combine this position with working at the clinic.

We use Take Care and Lifecare for medical record keeping.

Employment type:

Permanent position, Full-time, no evenings or weekends. Probationary period may apply.

Qualifications

We are looking for you who:

  • Are a registered nurse/advanced practice nurse
  • Have good knowledge of Swedish, both spoken and written
  • Have a B driver's license
  • Previous work experience in home care is highly meritorious
  • Good medical competence
  • Are structured
  • Solution-oriented

The position will be filled provided it does not need to be taken for internal relocation, according to current regulations.

In patient-related work within the Stockholm County Council (SLSO):

- register control is applied before the decision on employment according to SLSO's overarching guidelines for register control at employment.

- employees who will have patient contact must undergo a new employment examination, according to current directives, which is done through occupational health services before employment.

- requires, in addition to Swedish professional licensure, good knowledge of the Swedish language corresponding to level C1 according to the Common European Framework of Reference for Languages (CEFR). The same requirements are guiding for non-licensed personnel with patient contact.
