RESPONSIBILITIES

Here, you will become an important part of a dedicated team working to provide secure, safe, and person-centered care to the residents of Gävle. No two days are alike, making the work both developing and stimulating.

Job Duties

In this position, you will work within two home healthcare areas in Home Healthcare North. The assignment does not involve having your own care responsibility for a specific geographical area. Instead, the focus is on supporting operations where needs arise and contributing to a flexible and well-functioning healthcare organization. Together with nurse colleagues, auxiliary nurses, home help staff, and other professionals, you contribute to high-quality care.

The assignment includes:

• Carrying out planned and unplanned health and medical care interventions.
• Conducting independent medical assessments.
• Planning, implementing, and following up on health and medical care measures.
• Prioritizing interventions and making decisions based on the patient's current needs.
• Making assessments during home visits, at care and nursing homes, and via telephone.
• Delegating, supervising, and supporting care staff.
• Providing advice and support to patients, relatives, and employees.
• Documenting according to current legislation and guidelines.
• Working to promote health and prevent illness with the patient's best interests in focus.
• Collaborating with internal and external actors to create secure and cohesive care.

QUALIFICATIONS

We are looking for you who:

• Are a licensed nurse.
• Hold a valid driver's license (Category B).
• Have good knowledge of Swedish, both spoken and written.
• Are computer literate and can document in digital journal systems.
• Feel confident in making medical assessments and working independently.

Experience from municipal health and medical care, home healthcare, or elderly care is considered an advantage.
