About the Role
The Palliative Medicine unit is a county-wide operation with locations in four municipalities: Sollefteå, Härnösand, Sundsvall, and Örnsköldsvik. The Palliative Medicine team in Sundsvall consists of four full-time nurses. We work together with doctors in a team and care for patients in their homes in collaboration with home care nurses. The unit is located at Sundsvall Hospital, with an office at Ånge Health Centre. The role involves working independently, being flexible, and collaborating both within and outside the team, as well as with the patient's relatives. We are looking for a Specialist Nurse. The position is a permanent full-time employment with start date according to agreement. Location: Sundsvall.
Responsibilities
In addition to standard nursing duties, specific tasks within palliative care include, for example:
- Responsible for follow-up of activities/goals in the SIP (Coordinated Individual Plan).
- Responsible for ensuring that relatives receive adequate support based on needs.
- Collaborate with the patient's relatives in shaping the patient's care.
- Provide support to colleagues.
Qualifications
We are looking for someone who:
- Is a licensed nurse with specialist education.
- Has work experience as a nurse.
- Has good knowledge of Swedish, both spoken and written.
- Has good knowledge of English, both spoken and written.
- Has a valid B-class driver's license.
It is meritorious if you:
- Are educated as a District Nurse.
- Have work experience as a nurse working in palliative home care.
- Are educated as a Specialist Nurse in Palliative Care.
