Stadsviken Health Centre is one of the largest in the county, with approximately 15,500 registered patients. We are a team of around 60 employees working forward-thinking and in close collaboration to develop our methods for the benefit of our patients and to create a good working environment.
We are looking for
We are looking for a registered nurse who can become a care coordinator for our psychosocial team. Training as a care coordinator is an advantage; otherwise, there is an opportunity to undergo this training within the role.
Personal qualities are highly valued. As a person, you are secure and able to work independently. You lead the team with engagement and have a positive attitude. You are proactive and enjoy working with development. Experience from psychiatry is an advantage.
What you will work with
The team consists of CBT therapists, and occupational therapists and physiotherapists are available as needed. The team has weekly team rounds where you, as the care coordinator, play a key role. You lead and distribute incoming cases in close collaboration with the rest of the team. You are the first point of contact for the patient, conducting assessments, medication follow-ups, and, to some extent, supportive conversations. In addition, you have telephone hours and communicate via chat. Each week, the team holds rounds together with doctors at the health centre, and every third week together with doctors from psychiatry.

Information about the position
We offer you a permanent full-time position, daytime hours. Start date by agreement. Region Norrbotten applies individual salary setting according to collective agreement.
