We have conducted a development project in the department that has resulted in new working methods and routines that we now follow. This includes close collaboration with physiotherapists, occupational therapists, dieticians, social workers, pharmacists, and especially stoma therapists to ensure that care around patients is as good as possible. To promote safe discharges for our patients, we have a care planning nurse working with us.
