Hälsocentralen Stöde is a smaller and well-functioning unit with about 2,700 registered patients. Here, regular and experienced staff work in a stable environment. We are located about 40 km west of Sundsvall with good communication via both bus and train.

With us, you will work alongside a competent and close-knit team consisting of family doctors, training doctors, district nurses with different specialties, nursing assistants, social workers, psychologists, laboratory staff, medical secretaries, physiotherapists, and sick leave coordinators. We are now looking for a district nurse/diabetes nurse to join our team! The position is a permanent full-time employment, with start date according to agreement.

District Nurse/Diabetes Nurse at Hälsocentralen Stöde Job Responsibilities

As a district nurse/diabetes nurse with us, you will primarily work with:

• Your own clinic
• Diabetes care
• Telephone counseling
• Preventive health work

You will encounter a wide range of patients - both acute issues and chronic conditions such as asthma, COPD, diabetes, dementia, and cardiovascular diseases. We work flexibly with both physical and digital visits and have close collaboration between professions, always with the patient in focus.

We are looking for someone who

• Is a trained district nurse or diabetes nurse, alternatively a licensed nurse with work experience in diabetes care
• Has good knowledge of Swedish in both spoken and written form

It is an advantage if you

• Have work experience in primary care
