Home Visit Nurse at Sandviken Norra Din Hälsocentral

Sandviken Norra Din Hälsocentral is one of Region Gävleborg's larger health centers with approximately 11,000 registered patients. We have around 50 employees at the health center, including doctors, nurses, district nurses, nursing assistants, occupational therapists, physiotherapists, a psychosocial team, medical secretaries, and rehabilitation coordinators. We have specialist clinics for asthma/COPD and diabetes with an acute flow where we meet primary care urgent patients.

The operation will become part of Region Gävleborg Din Hälsocentral AB, the company for publicly run health and child care centers in Gävleborg. The transition will take place on February 1, 2026. Together, we want to develop and shape primary care and create a good working environment for managers and employees.

We are now looking for someone who wants to contribute to the development of our home visit operations and work independently with safe and person-centered care in the patient's home environment.

JOB RESPONSIBILITIES

As a home visit nurse, you will work with patients in their home environment and perform care interventions that would otherwise take place at the health center. In your work, you will use Lifecare for communication and documentation between different healthcare providers and work independently with support from colleagues in the organization.

Your main responsibilities include:

• conducting home visits to patients
• performing nursing interventions such as medication administration, dressings, as well as checks and follow-ups.

QUALIFICATIONS

We are looking for someone who enjoys working independently as a home visit nurse and is motivated to contribute to the development of our operations close to the patient. You have a good ability to plan and structure your work and can prioritize your tasks effectively. You are flexible and handle variations in work pace and content while maintaining focus on quality and patient safety. You see collaboration as a natural part of your working method and contribute to a good working climate through clear communication and a professional approach towards both colleagues and other healthcare providers. With a service-oriented attitude, you meet patients with attentiveness and respect, contributing to a safe and positive care experience.

In the role of home visit nurse, you must:

• be a licensed nurse.

For this position, it is advantageous if you are a trained district nurse, have experience from primary care, or have experience in home healthcare.
