Rehabilitation Coordinator at Hofors Din Hälsocentral
Hofors Din Hälsocentral provides standard primary care with doctors, child health center (BVC), district nurse and nursing reception, assistant nurse reception, sampling, psychosocial team, occupational therapists, physiotherapists, rehabilitation coordinators, and medical secretaries. The premises also include a maternity care center. We have about 40 employees in our unit and we hold regular professional group meetings.
We work to make good care even better and focus on what is valuable for patients; from medical quality to good opening hours and a warm welcome. Our health center aims to provide safe care when someone needs it and works according to our guiding principles Always-Close-Safe.
The operation will become part of Region Gävleborg Din Hälsocentral AB, the company for publicly run health and child health centers in Gävleborg. The transition will take place on February 1, 2026. Together we want to develop and shape primary care and create a good working environment for managers and employees.
JOB DESCRIPTION
The role of rehabilitation coordinator includes coordinating efforts for sick-listed patients in need of support for work-oriented rehabilitation. You will collaborate with and support doctors, other professional categories, patients, and the unit manager in the sick leave process. You will also be the unit's contact person for relevant parties in the sick leave process.
As a rehabilitation coordinator, you will work with:
- sick leave cases
- being the bearer of knowledge about the regional work on developing the sick leave process
- participating in network meetings for rehabilitation coordinators and professional development organized by process leaders within Region Gävleborg.
The assignment is based on the coordination law that came into force on February 1, 2020, and includes three areas that you as a rehabilitation coordinator will work with:
- Personal support for sick-listed patients who need it to return to work or enter the labor market.
- Internal coordination within the unit and between other units for a shared understanding in the individual patient's sick leave and rehabilitation process, and contribute to the establishment of a rehabilitation plan.
- Collaboration with other actors by contributing the healthcare perspective to facilitate their efforts within their area of responsibility, and being the coordinator for the unit's Trisam team, which is a structural collaboration between the Employment Service, the Social Insurance Agency, the municipality, and healthcare.

QUALIFICATIONS
We are looking for someone who is flexible with a solution-focused approach. You see opportunities in changes and can adjust your priorities when needed. You care about and want to help others, and you find it easy to make contact and develop new relationships with people you meet. You enjoy collaborating with others, but the role also requires you to work independently. We expect you to take your own initiatives and want to be involved in developing the role. You are meticulous and have a structured way of working with an eye for detail. Personal suitability is of great importance.
In the role of rehabilitation coordinator, you should have:
- a university degree in medicine, psychology, or social work such as a licensed occupational therapist, licensed physiotherapist, social worker, or licensed nurse
- basic knowledge of medical terminology
- experience in reading and documenting in medical records or equivalent
- a valid B driver's license.
