The primary care includes 21 publicly operated health centers, 1177 Vårdguiden, Primary Care Rehab, the Primary Care Health Unit, Family Health, and A Path for Children and Adolescents with Mental Health Issues. Our mission is to create good conditions for our patients so that the residents of Kronoberg receive the right care and treatment from the best possible competence at the right time and at the right level of care.

At Primary Care Rehab, the rehabilitation resources of primary care are gathered. Here, occupational therapists, physiotherapists, psychosocial resources, rehab assistants, and rehab coordinators work together to assess and treat mental and physical health issues at the county's public health centers.

We are now looking for a colleague to join our team at the health center in Lessebo. At the health center, there is a high willingness to collaborate and trust in each profession's competence. There is a good staffing of doctors, many of whom have been in Lessebo for a long time. With us, the patient is at the center, and we always do our best to ensure that our patients receive quality care and feel secure and trusted through good treatment. The health center is centrally located in Lessebo, within walking distance to both the bus and train station.

We value professional development and will work with you to create an individual development plan to ensure you have good conditions in your work and the opportunity to develop over time. You will also be offered supervision.

JOB DESCRIPTION

As a psychosocial resource within Primary Care Rehab, you will work with assessment and treatment. Your role involves helping both those who need a single support conversation and those who require longer treatment. In primary care, there is a wide range of patients, and you will encounter patients with anxiety, depression, crises, relationship problems, and stress issues. The target group is individuals over 18 years old. We offer both physical meetings and digital contacts with our patients.

You will collaborate with both your rehab colleagues and other professionals at the health center to help the patient in the best way possible and provide good and accessible care. You are an important part of the health center's rehabilitation team, where you will collaborate and plan treatment for patients who are on ongoing sick leave.

QUALIFICATIONS

To be considered for this position, you should be a psychologist, psychiatric nurse, social worker, or have equivalent higher education. You should also have treatment competence corresponding to level 1.

It is an advantage if you have experience working in healthcare, preferably in primary care.

As a person, you have good planning skills and demonstrate both initiative and responsibility. You thrive in teamwork and value collaboration between different professions. Furthermore, you work with the patient's best interests in focus and contribute to a good working environment.

Great emphasis will be placed on personal suitability.
