Social Welfare Officer for the Emergency Department

We are now looking for a social welfare officer for the emergency department in an exciting one-year project, with the possibility of a permanent position.

This position is a new initiative within the social services department and will be based in the emergency department. As a social welfare officer, you will work independently with processing, follow-up, and evaluation of interventions according to the Social Services Act. You will be part of the municipality's discharge support team but will have your workplace in the emergency department, where you will closely collaborate with healthcare staff to ensure a safe and smooth transition from hospital care to municipal care.

RESPONSIBILITIES

In this role, you will investigate and decide on interventions for patients in the emergency department and work independently in an environment where quick decisions are required. You will document in Life Care and actively contribute to the development of the operations through working groups and specialist areas.

There is a possibility for the position to become permanent. Working hours are scheduled during weekdays from Monday to Friday.

QUALIFICATIONS

You who are applying are a social worker or have another education that the employer deems equivalent. We want you to have good knowledge of the relevant legislation and experience in processing according to the Social Services Act. Experience in processing within the elderly target group is an advantage.

Since communication and documentation are part of the job, you need to communicate well in Swedish, both verbally and in writing. Experience with processing in the Life Care system is an advantage. The job requires you to have a B driver's license for manual transmission.

As a person, you are independent and development-oriented with the ability to take responsibility. You find it easy to collaborate and communicate in speech and writing. You should also have decision-making ability and the ability to translate goals and visions into practical action.

We prefer that you can make quick and clear decisions both independently and together with others. Initiative is also an advantage. Being unpretentious with the ability to work with other people, to listen, show empathy, and respect is seen as positive. We also value that you are good at creating and maintaining good relationships, both internally and externally.

OTHER

The operations of the social services department are aimed at the elderly and individuals who need functional support as well as those in need of home healthcare. The operations are functionally divided into four areas: support in ordinary housing, support in care and nursing homes, health and medical care, and functional support. The department also includes the authority unit, national gymnasium, and assistive technology center in Eastern Skåne.
