About the position
In our internal medicine department, we care for patients with nephrological and endocrinological diseases. We have both planned and acute admissions. Our nephrological focus is unique as we are the only department in Southern Stockholm that performs peritoneal dialysis. Therefore, you will gain specialized skills in performing peritoneal dialysis manually and with machines. Our patients may also require surgical interventions during their stay with us, which means you will also gain experience and knowledge in surgery and orthopedics. Our goal is to help and support individuals living with lifelong treatment and chronically ill patients to maintain or improve their quality of life. Our patient flow in endocrinology includes, among other things, patients with newly diagnosed diabetes or diabetics with complications such as diabetic foot ulcers or ketoacidosis. A cornerstone of our work is to collaborate in a team with the patient and their relatives to provide the best care with the patient in focus.
We are characterized by a lack of prestige and have close cooperation between different health professions as well as partners within and outside the hospital. As a Coordinator, you will have your own responsibility in leading the planning of our patients who need support with planning and actions before discharge. We are looking for 2 colleagues to work 50% in the coordination assignment.
The position involves working clinically and contributing to our 24/7 operations by working weekends. We work 2 out of 5 weekends.

Qualifications
Requirements:
- Licensed nurse.
- Experience coordinating discharges to other healthcare providers.
Meritorious:
- Experience in nephrology, endocrinology, dialysis, or diabetes.
