The operation is organizationally part of "Close Care and Health," Mobile Teams, and Specialized Palliative Care, Southwest Scania. ASIH offers specialized palliative care and other advanced home healthcare to patients of all ages residing in the catchment area of Malmö Municipality. Care is provided in the patients' own homes or in specialized housing.
Patient needs determine the visits; we perform medical and nursing assessments and interventions. We work independently but are part of interprofessional teams consisting of doctors, nurses, social workers, physiotherapists, occupational therapists, assistant nurses, psychologists, dietitians, and medical secretaries. We have our own duty line, and the operation's doctors are available 24/7.
ASIH has full medical responsibility for enrolled patients and works with holistic, person-centered care, which is a prerequisite for providing dignified end-of-life care. ASIH provides specialized care but collaborates with home healthcare services when needed and with home help services for basic nursing. We opened three teams in March and have a total of 60 care spots. Additionally, we have an outpatient team consisting of a palliative clinic and a rapid response team that provides rapid interventions for patients not enrolled in ASIH.

RESPONSIBILITIES
The enrolled patients require specialized palliative care, meaning they have a complex symptom picture or their life situation entails special needs. This can range from injections, infusions, medication pumps, catheter changes, paracentesis, oxygen therapy, and wound care to supportive conversations, support for relatives, or assistance in contacting authorities.
All professions meet in the teams in the morning to review the day's planning. We perform mostly planned home visits during daytime. The afternoons are then used for documentation, reflection rounds, team supervision, individual reflection, and development work. In the evenings and nights, nurses are on duty across the teams. The phone to ASIH is open 24/7, and we constantly assess and make visits to patients who need it. In addition to daily work, as a nurse, you conduct admission interviews together with doctors, participate in assessments prior to potential admissions, and work continuously with support for relatives and bereaved families. We work day, evening, and night shifts.
As a new nurse with us, you will receive at least four weeks of side-by-side training. We use an introduction program developed at the unit, and after the side-by-side training, a mentorship follows where you receive support from a specialist nurse in palliative care, according to your needs. We value development work and further education for both new and experienced staff. ASIH has a close collaboration with the palliative care ward, which has 12 inpatient beds. Furthermore, we collaborate with the hospital and various clinics.

QUALIFICATIONS
We are looking for you who are a licensed nurse with permission to practice in Swedish healthcare, preferably with a few years of professional experience. Language skills in Swedish equivalent to at least level C1 according to the Council of Europe's scale are required. Since the operation consists of care spots in the patient's own home or specialized housing, a valid B-driver's license for a manual transmission car is required for the position. We would appreciate if you have specialist education in palliative and/or oncological care.

Part of the work involves support and conversation, so it is important that you are communicative and have the ability to meet people on different levels. Furthermore, you have a flexible approach and can prioritize between different tasks when the work pace is high.

The Department of Close Care and Health has existed since January 1, 2025. The department is responsible for grant-funded care operations within close care. This includes, among other things, mobile teams and specialized palliative care, outpatient operations within close care, acute care within close care, 1177 by phone, as well as some region-wide functions. Additionally, the department is responsible for the development and follow-up of the transition to close care in Region Scania. The number of employees is estimated to be around 1,000.
