A varied and stimulating job as a care coordinator nurse, where you work in a team with the patient and other professions. You will also carry out point interventions and acute assessments. The position is a permanent full-time employment with a working time of 38.25 hours/week and daytime shifts every 5th weekend. You will receive support from an experienced team and have the opportunity to influence and develop the operations.

Who are you?

Requirements:

• Registered Nurse
• Good knowledge of Swedish, both spoken and written
• Good computer skills (we document in Procapita)
• Category B driver's license for manual transmission cars

• Specialist training in elderly care or as a district nurse

About us

Within the care administration, we provide care, support, and care to people who are elderly, have a disability, or need home care. We also offer health-promoting measures and support to people who care for and support a relative. Our goal is that those who need care and support in their daily lives should be able to live an independent life as far as possible.

The Health and Medical Department is responsible for interventions within home care and rehabilitation. The interventions are carried out by nurses, assistant nurses, rehabilitation assistants, occupational therapists, and physiotherapists, who also delegate to care personnel in other areas of operation based on profession.
