Bräcke Diakoni is looking for a registered nurse for our home care team in Linköping. The position is 80%. Here you work during daytime hours, weekdays, in a role where you meet patients in their home environment and contribute to close and integrated care.

Your Role

With us, you contribute to safe and secure care for our patients in their home environment. You meet them in ordinary housing, at Bräcke's security housing facilities, and within our home care areas in City Syd. Many of our patients are elderly or have disabilities. A key part of the role is therefore to monitor health over time, notice changes early, and contribute to interventions that strengthen independence and quality of life.

You are based at Ramstorp Security Housing, where you share an office with your nursing colleagues. In your work, you supervise care staff, collaborate with doctors and rehabilitation personnel, and contribute the nurse's perspective to the development of our working methods. You become part of a cross-professional context where knowledge within nursing, rehabilitation, palliative care, and cognitive illness is utilized around the patient.

Who We Are Looking For

We are looking for you who are a registered nurse and thrive in a role where you collaborate broadly and contribute your expertise. You have the ability to create an overview and plan care interventions that are health-promoting and adapted to the patient's situation. You are accustomed to guiding care staff and contributing to joint prioritizations when needs change. Your approach is clear and attentive, and you contribute to a co-creative working approach around the patient.

Your Qualifications

• Registered Nurse (Legitimerad sjuksköterska)
• Experience with HSL duties (Health and Medical Services Act)
• Valid driver's license (Category B)
• Fluent in Swedish (both spoken and written) and proficient in computer skills

Meritorious Experience

• Experience in home care
• Experience in supervising staff in nursing work
• Experience in geriatrics or palliative care
• Experience with the medical record systems Treserva and Cosmic

Bräcke Home Care – Linköping City Syd

In Linköping City Syd, Bräcke Diakoni has overall responsibility for care, home care, and rehabilitation at home. Here, approximately 150 dedicated colleagues work together in our operations.

In the home care team, nurses, physiotherapists, and occupational therapists work closely together, with the patient's daily life and living situation as the starting point. Around the patient, we also collaborate with Bräcke's home care groups, doctors, the municipality, and other healthcare contacts. This provides good conditions for creating integrated care and support, where different competencies are utilized. As a faith-based actor, we are driven by the desire to make a real difference for those we serve and to develop our work together – close to the patient and close to each other.
