The Social Services Department is now seeking a physiotherapist with a coordinator role for the Rehabilitation Unit of Home Care. In this role, you will become an important part of our multidisciplinary coordination team, working together with nurses, social workers, and home care coordinators to ensure a safe and seamless transition from inpatient care to the home environment.

Here, you will have a central role where you combine your physiotherapy competence with coordination, planning, and close collaboration with patients, relatives, and various actors within the healthcare sector.

Job Responsibilities

The coordination team is responsible for creating an efficient and sustainable discharge process where the individual's need for security, continuity, coordination, and safety is the focus. The team handles all parts of Lifecare from admission notification to discharge.

As a physiotherapist with a coordinator role, you represent the rehabilitation unit in the team and serve as a vital link between different operations and professions.

In the role, you will among other things:

• Represent the rehabilitation unit in the multidisciplinary coordination team.
• Work with planning and coordination of discharge processes in Lifecare.
• Collaborate with patients, relatives, colleagues, and actors within inpatient and primary care.
• Serve as the entry point to the rehabilitation unit of home care.
• Process and make decisions regarding home care according to delegation.
• Staff the rehabilitation phone and coordinator mailbox, as well as distribute incoming cases.
• Contribute to internal collaboration and development of the operations.
• Participate in clinical physiotherapy work together with colleagues within home care, as needed and possible.

The work is scheduled during daytime hours, Monday–Friday.

Qualifications

We are looking for you who:

• Are a licensed physiotherapist.
• Hold a valid driver's license (Category B).
• Thrive in a coordinating role with many contact surfaces.
• Are accustomed to working independently while appreciating close teamwork.
• Have a professional and respectful approach.
• Are structured, flexible, and solution-oriented.

Merits

It is an advantage if you have:

• Experience working in home care or similar operations.
• Experience with Lifecare.
• Experience in administrative, coordinating, or logistical work.
