Now, Tingsryd Health Center is looking for a specialist in general medicine who wants to work as a home care physician, with duties both in ordinary housing and in special housing (SÄBO).

RESPONSIBILITIES

As a general specialist responsible for home care, you are responsible for the medical assessment, follow-up, and treatment of patients registered in home care as well as those residing in SÄBO. The patient group consists mainly of elderly people with complex care needs and multimorbidity.

The work involves regular home visits and rounds at special housing facilities, where medication reviews and advanced care planning are part of everyday life. This places demands on your clinical experience, and you need to be confident in independent medical assessments.

You collaborate closely with the health center's coordinator in the planning, coordination, and follow-up of patient care, as well as in close dialogue with the municipal nursing organization, care staff, and other professions. The work is characterized by continuity, a holistic view, and good collaboration.

Examples of other tasks include collaboration with relatives and inpatient care when necessary, as well as participation in palliative care and end-of-life care.

QUALIFICATIONS

We are looking for you who have specialist competence in general medicine. Furthermore, we see specialist competence in geriatrics, or documented experience of working in home care as meritorious. A B-class driver's license is a requirement for this position.

We also place great importance on personal suitability. As a person, you are sensitive, clear in your communication, and have a good ability to structure and prioritize. You are team-oriented, empathetic, and of course have a genuine interest in primary care. We value that you can meet different people based on their needs.
