About the Department

Primary Care is a county-wide organization divided into four areas, one of which is Örnsköldsvik, comprising the six health centers in the municipality. Hälsocentralen Ankaret is the county's largest health center and is centrally located in Örnsköldsvik. Approximately 65 employees from various professions work at the health center.

As a district nurse/nurse in this assignment, you will work to develop "Nära Vård" (Close Care), adapting working methods for our patient group of the most frail elderly, as well as for patients who seek us frequently. We see that the work will involve, among other things, continuing to drive processes to develop our working methods.

The position is a full-time temporary assignment with a start date to be agreed upon, ending on 2027-03-31.

Responsibilities

• Develop working methods within "Nära Vård" for our patient group of frail elderly and patients who seek us frequently.

Qualifications

We are looking for you who

• Are a licensed nurse or district nurse.

It is an advantage if you

• Have previous experience working in Primary Care or with elderly patient groups.
• Have experience working in the medical record system Cosmic.
