The Social Administration in Örkelljunga municipality consists of social services, elderly care, municipal health care, care for people with disabilities, and preventive functions. We are organized into a preventive unit, a supervisory unit, and an execution unit. Our guiding principle is teamwork, utilizing the strength of the small municipality to work closely together in short decision-making chains for an efficient organization and a service-oriented perspective.
Dementia care in Örkelljunga municipality is a prioritized area within care and support. The care is conducted according to the National Guidelines for care and support for dementia diseases set by the National Board of Health and Welfare. The goals for dementia care in Örkelljunga municipality are based on these guidelines for good care and support: individualized, knowledge-based and safe, accessible, equitable, and effective.
The demographics indicate that the number of elderly people is increasing, including those who develop permanent cognitive impairment/dementia. In Örkelljunga, we aim to offer equitable care to this group, allowing individuals to live at home as long as possible while ensuring continuity and security for them and their relatives.
The position is placed under the preventive unit and collaborates broadly with various operations, each with different focuses. The dementia nurse has a coordinating role in health-promoting and preventive efforts aimed at seniors.
RESPONSIBILITIES
The assignment includes, among other things:
- Support for people with dementia
- informing about diagnosis, symptoms, and progression
- helping the patient and relatives create strategies and routines in daily life
- conducting advisory home visits and offering individual support
- Support for relatives
- organizing meetings and training for relatives
- explaining behavioral changes and providing advice on interaction
- supporting relatives in navigating care and finding the right services
- Support for staff
- supervising and developing the skills of various staff groups
- providing guidance in complex situations
- training in cognitive diseases, interaction, and BPSD work
- Collaboration with the region
- participating in dementia investigations and care planning
- collaborating with health centers and memory clinics
- being part of collaboration teams and networks locally and regionally
- Development and improvement work. The role also entails responsibility to:
- monitor research and national guidelines
- drive quality development within the municipality's dementia care
- participate in training, lectures, and development projects
QUALIFICATIONS
We are looking for a licensed nurse who is either trained in the Silvia method or a specialist nurse in dementia.
The ideal candidate is organized and structured. They easily collaborate and have the initiative to see what needs to be done and in what order. The assignment opens many doors, and you have the ability to contribute to the development of dementia care in the municipality. It is important that you are self-sufficient and can take your own initiatives in dialogue with the responsible manager.
You need to be confident, trustworthy, and able to motivate and inspire.
