The Primary Care Area West consists of the Health Centers in Sollefteå, Kramfors, Nyland, Höga Kusten, and Ramsele/Junsele. In the role of nurse with us, you will be part of the psychosocial team with a coordinating function for our patients with mental health issues as well as in outpatient work.

We are now looking for a nurse for the Psychosocial Team. The position is a permanent full-time employment with a start date according to agreement.

Nurse for the Psychosocial Team, Sollefteå/Kramfors

• Medication follow-ups and outpatient work
• Active participation in collaboration with doctors, rehabilitation teams, and addiction teams
• Manage incoming referrals
• Participate in case conferences
• Telephone counseling
• Follow up self-assessment forms

Your efforts will enable us to increase accessibility and provide our patients with good and close care. Your placement will be at either Kramfors or Sollefteå Health Center, as agreed.

We are looking for someone who

• Is a licensed nurse
• Has good knowledge of Swedish in both spoken and written form
• Has experience with community support functions

It is an advantage if you

• Have specialist training in psychiatry
• Have experience working as a psychiatric nurse
