The Role of Rehabilitation Coordinator
As a rehabilitation coordinator, you work to create good conditions for our patients' return to work and to optimize the sick leave period for the patient. As a rehabilitation coordinator, you collaborate internally with the remaining team on individual cases, but also on development work regarding rehabilitation and sick leave. Collaboration also takes place externally with parties such as authorities, employers, relatives, and doctors. You are also part of the collaboration forum together with the municipality and other actors at both the individual and group level.
The role is a permanent position at 100% with working hours Monday to Friday, daytime. Some time may be shared between our other clinics in Nässjö, Vetlanda, and Tranås. Start date is to be agreed upon.
Your Future Workplace
At the Psychiatric Clinic Höglandet, we have about 190 employees. In Eksjö, there are clinic-wide units such as referral and assessment units, emergency reception, and care departments. The clinic has outpatient clinics in Eksjö, Nässjö, Tranås, and Vetlanda. The clinic's resources are distributed so that we take into account both the clinic perspective and the regional psychiatric care processes. We participate in the development and implementation of joint care programs and psychiatric care processes in the region.
The position in question involves placement at the reception in Eksjö. We have about 20 employees at each reception, with professions including specialist doctors, psychologists, psychotherapists, social workers, specialist nurses, occupational therapists, caregivers, and administrative staff.
Your Profile
We are looking for someone with a basic university education in health and medical care or social work, such as a social worker, occupational therapist, nurse, or with other education we consider equivalent. Since the role involves travel, you must have a valid driver's license (B).
If you have experience working as a rehabilitation coordinator or have completed training as a rehabilitation coordinator, it is an advantage, but not a requirement to apply for the position. Experience from previous work with collaboration between municipalities and other authorities is seen as an advantage, as well as experience working with people with mental health issues. To thrive in this role, it is beneficial if you are solution-focused. It is important that you can work independently and take initiative, but at the same time have an easy time collaborating with others and possess good communication skills. We place great emphasis on personal suitability.
