District Nurse at Strömsbro Health Center

Strömsbro Health Center currently has approximately 11,100 registered patients. We provide standard primary care with medical consultations, child health care, a psychosocial team, occupational therapy, physiotherapy, and home visits. We also have specialist nursing consultations for asthma/COPD, incontinence, and diabetes. With a safe and warm approach, we deliver good and close care, tailored to each unique patient. We are part of Min vård Gävleborg, which offers digital healthcare services, an important piece in creating more accessible and equitable care.

RESPONSIBILITIES

As a district nurse with us, you will work closely with all professional roles at the health center. You will play an important part in supporting, treating, and guiding people towards a healthier life. You will, among other things, work in our home visit group with the discharge process in Life Care and home visits.

As a district nurse, you will work:

• with home visits, your own clinic, and telephone advice
• in our emergency team together with doctors and nursing assistants.

QUALIFICATIONS

We are now looking for someone who wants to work with us to provide our patients with good and close care. You are used to working independently, are confident in your role, and are interested in developing and evolving in your professional role. To thrive at work, it is important that you enjoy challenges and want to be involved in the development of the health center. You easily adapt to changing situations and see the importance of contributing to a good cohesion in the workplace.

In the role of district nurse, you should:

• be a licensed nurse with further education as a district nurse
• hold a valid B driver's license.

For this position, it is advantageous if you have experience working in primary care and if you have worked in Life Care.
