We are now welcoming a curator for a permanent position at Vårdcentralen Lindeborg and Vårdcentralen Kroksbäck. The position involves working 50 percent at each health center.

Vårdcentralen Lindeborg is a well-functioning and pleasant health care unit in southern Malmö, which has good contacts with other care providers in the district. We see collaboration as a success factor and are a health center that strives for openness and views communication as a natural working tool. Our daily work is characterized by patient focus and value-driven management, where operational development is a natural part of our working method.

Our health center is well-equipped in terms of both personnel and medical equipment. Today, we have 37 employees in various professional categories who take care of our more than 8,460 registered patients. We offer a complete range of services including medical consultations, district nurse consultations, psychologists, curators, physiotherapists, and occupational therapists. You will also find specialist clinics for heart failure, diabetes, asthma/COPD, and hypertension. Additionally, we have a child health center (BVC) and a training center for future specialist doctors with several training positions. Together, we work to offer all our patients safe and accessible primary care with a high-quality approach.

You can easily reach our bright and beautiful premises built in 2008. We are located next to Hyllie station, near the arena area and Emporia, with good commuting options to other parts of Skåne and Copenhagen.

Vårdcentralen Kroksbäck has been around since 1986 and is one of Malmö's oldest health centers. We have about 7,000 registered patients and are over 25 employees with a well-functioning internal collaboration both between and across professional boundaries. The health center has a stable medical staffing, a good work environment with flexible working hours, where participation and influence are self-evident for us. We have a strong focus on improvement in our operations and value our employees' involvement in the development of the organization. Adjacent to the health center, there is Rehab Kroksbäck and the Child Health Center Hyllie.

We are in the process of implementing team rounds according to the Alaska model. The Alaska model is a workplace reform that results in good and close care. The work team is divided into three cross-professional teams with different professions in each team where the patient is included in the team. JOB RESPONSIBILITIES

With us, your task will be to investigate and meet our patients' needs for psychosocial support through, for example, supportive and/or therapeutic conversations. You will assess the individual's psychosocial ill health from a holistic perspective and treat patients with various mental and social health conditions based on evidence-based treatment methods. The work takes place both individually and in groups. Furthermore, it includes being active and contributing to the development of the organization through collaboration with various care providers, both internally and externally, as well as with other important parties for the patient.

QUALIFICATIONS

We welcome you who have a recognized degree in social work and have previous experience working as a curator. It is highly meritorious if you are a licensed health and medical care curator with authorization to operate within Swedish healthcare. If you are a licensed health and medical care curator, it is a requirement to have language skills in Swedish at least corresponding to level C1 according to the Council of Europe's level scale. It is advantageous if you have work experience in psychosocial treatment work, crisis and grief processing, and/or rehabilitation coordination.
