JOB RESPONSIBILITIES
Are you a nurse with experience in palliative care who wants to try something new? Now you have the chance to try the position of hospice case manager. The hospice case management is located at F O Petersonsgata in southwestern Gothenburg and consists of two regular case managers with extensive experience. We are now looking for a colleague who can strengthen our case management team for six months. With us, you will have a meaningful and important job where you can make a difference for those we are here for.
You will handle applications and assess whether the patient is entitled to a hospice place based on the health care process. You will decide and allocate places in three hospices, one of which is within Gothenburg City’s own management and two external.
You will collaborate with several parties and gather information to get a comprehensive picture to support your decisions. Home visits to patients may also be part of the investigation work. You will follow up on your work and the occupancy of hospice places. We have clear assessment criteria that we base our work on, and you will have experienced colleagues in the team to consult with if needed.
The role requires good communication skills and the ability to make independent decisions in your work. This position is an opportunity for you to challenge yourself and develop in your profession. QUALIFICATIONS
We are looking for you who are a licensed nurse with experience in palliative care. We would like you to have experience in case management or assessments of a similar nature, but it is not a requirement for the position.
You who apply are secure in your work role, not influenced by others in your decision-making, and can stand by the decisions made. You take responsibility for your and the group’s tasks. You do your best to complete tasks and follow up on the results.
You work well with others by listening, showing interest, and respect. You understand the common mission and work to make it as good as possible. Furthermore, you have the ability to convey information clearly. You have many points of contact and have a good ability to adapt your communication based on whom you meet.
