The work is led by a first-line manager, who, together with the operations manager and other first-line managers in primary care, forms the management group of the organization. Care Coordinator/Coordination Nurse for Smedjebacken Health Center

Job Responsibilities

The position consists of two parts:

The first part involves working as a care coordinator/coordination nurse. As a care coordinator, you will work independently with conversations and assessments of patients with depression and anxiety disorders.

The second part involves working with substance abuse/risk use and dependency. The work includes health-promoting activities with discussions about lifestyle habits and screening for potential risk use, substance abuse, and dependency. Assisting with medication misuse. Providing support during sick leave and care planning. Health conversations, sampling, and medication when necessary are also part of the job responsibilities.

You will have a central role in a team working with patients with mental health issues. Your team consists of primary care physicians, conversation therapists, rehabilitation coordinators, and psychiatrists. You want to work with your own practice and are interested in working with the above patient groups. You need to be able to work independently and in a team, take responsibility for your assignment, be flexible, and take initiative. We value a positive attitude and the ability to see opportunities instead of obstacles. Personal suitability is highly emphasized.

Requirements:

• Licensed Nurse.

Meritorious:

• You should have good collaboration and communication skills, as the job involves a team-leading role.
• Ability to maintain an effective and professional approach in meetings with patients, team members, and other professional categories.
• You should be able to work independently and be confident in patient conversations related to mental health issues.
• Ability to be creative, flexible, and engaging in patient meetings and as a team leader.
• Previous experience in psychiatric work as well as work with risk use and dependency conditions.
• Experience in developing individual care plans.
• Previous experience from conversation clinics is a plus.
• Interest in learning shorter conversation treatments, such as Behavioral Activation (BA) and Motivational Interviewing (MI).
• Familiarity with common conversation treatments (CBT, ICBT, PDT, (Mindfulness) and their content.
• Knowledge of the most common medication treatments for depression and anxiety disorders.

Personal suitability is highly emphasized.
