Job Description

As a nurse in the on-call team, you provide quality primary care to patients aged 18 and above in their home environments. Services are delivered in care and nursing homes, ordinary housing, short-term care facilities, and LSS (support and service for persons with certain disabilities) housing throughout the municipality.

During on-call hours, two nurses work in the municipality, planning and prioritizing interventions based on the patients' current needs. The work is independent and varied, requiring you to make confident decisions even in rapidly changing situations. Examples of tasks include symptom relief in end-of-life care, assessments during sudden health deterioration, and other acute or planned health care interventions.

You collaborate closely with patients, relatives, care staff, colleagues, and other actors both within the municipality and the region. Together, you work to create safe, secure, and person-centered care where the patient's needs are always the focus.

In your work, you use digital tools that support planning, documentation, and communication. You also have support from colleagues and on-call doctors via telephone when the situation requires it.

Qualifications

We are looking for you who are a licensed nurse. If you have work experience from home care, elderly care, or dementia care, we consider it a great advantage. You should be able to express yourself clearly in both speech and writing in Swedish. Good computer skills are required as we work with several different digital systems. Experience with the medical record systems Viva and Cosmic is also meritorious.

A driver's license is a requirement.
