VOSS, part of the Outpatient Psychiatric Care, is a care and support coordination team working with individuals who have long-term disabilities caused by mental illness and require coordinated interventions from specialist psychiatry and municipal housing support. The Role of the Nurse
As a nurse in the Care and Support Coordination Team, you work with coordinated interventions based on the working model: Care and Support Coordination (VOS). The individuals we meet often have complex needs, and it is crucial to be able to see and meet each individual based on their specific conditions.
You work independently as a contact person in close collaboration with housing support and are part of a multidisciplinary team consisting of nurses, care coordinators, psychologists, occupational therapists, social workers, and physicians.
The position is a fixed-term assignment lasting 10 months from the start date. There may be an opportunity for extension. Start date as soon as possible or by agreement. Working hours Monday to Friday, daytime.

The Psychiatric Clinic in Jönköping is one of the county's largest clinics, offering both outpatient and inpatient specialist psychiatric care. The department has seven outpatient clinics, six inpatient wards, and a psychiatric emergency clinic offering acute assessments around the clock.
The clinic provides psychiatric specialist care for residents over 18 years old. Parts of the clinic also serve as a county resource responsible for psychiatric intensive care, addiction treatment, forensic psychiatry, and emergency services, handling acute cases within both adult psychiatry and child and adolescent psychiatry.
In outpatient care, we work with specialist interventions in neuropsychiatry, addiction treatment, psychosis care, general psychiatry, and psychiatric rehabilitation. We work in teams providing assessment, investigation, treatment, and rehabilitation interventions.

The Care and Support Coordination Team (VOSS) is a team that, based on the political goal of Equal Care, targets the most severely ill within psychiatric outpatient care at Ryhov. Patients are referred from the general psychiatric clinic and the Neuropsychiatric Clinic (NPT). The purpose is to ensure patients maintain contact with the care and support system, reduce the number and length of hospital stays in the long term, and improve patients' social functioning and quality of life.
Your Profile
We are looking for you who are a registered nurse and preferably have further education in psychiatry and experience in psychiatric care. A B-driver's license is required for the job as travel occurs within the role.

You need to be flexible and able to plan your interventions yourself. You are driven, have a desire to think new thoughts, and are resilient. You work according to a psycho-pedagogical approach, are confident in yourself, and are open to learning new treatment methods. You are engaged with patients and their relatives.
