We at Hässleholm Municipality have revamped our approach to "A Path into Home Care".

Your role as a nurse in "A Path into" is to be the first point of contact when patients apply for home care. You will receive incoming referrals and visit patients to assess their eligibility for home care. The role also involves monitoring "My Plans" (Mina planer) and participating in the assessment of the interventions the patient needs upon discharge. You will be a key part in creating a secure and coordinated care process for our patients.

A part of the position involves working clinically as a nurse within home care.

Working hours are daytime, Monday–Friday (40 hours/week).

Responsibilities

• Assessment of eligibility for home care via direct patient visits.
• Coordination and planning of interventions with the municipality, hospital, and primary care via "My Plans" for patients previously unknown to the municipality.
• Information to patients and relatives, documentation, and care planning.
• Clinical work in home care (part of the position).

The work is varied, involving both administrative tasks and direct patient contact through home visits.

Who are you?

• Registered Nurse (RN).
• Have a warm approach and a genuine interest in patient-facing work.
• Thrive in a coordinating role and in collaboration with multiple actors.
• Are confident in clinical assessments and can work independently.
• Good computer skills and proficiency in Swedish, both spoken and written.
• We prefer that you have experience documenting in the medical record systems Procapita and LifeCare, as well as experience with the systems My Plans, NPÖ, and Pascal.
• Hold a valid B-driver's license.

Meritorious:

• Experience in home care, geriatrics, inpatient care, or care planning.

Before employment, you must prove that you have the right to work in Sweden (Swedish citizenship, EU/EEA citizenship, or a valid residence and work permit).
