We are looking for a nurse to work both at the health center and in home care. You will also be responsible for the hypertension clinic. Here, you will be an important part of a team where care, quality, and collaboration are at the center.

With us, you will work with a wide range of tasks – from triaging and emergency care to counseling and follow-up of chronic diseases. You will meet patients of all ages and actively contribute to care that makes a difference.

As a nurse/district nurse, you will:

• Work with health and medical care, such as health checks, vaccinations, wound care, and chronic illnesses.
• Provide telephone counseling
• Work in the Take Care journal system
• Prescribe free consumables and medications
• Participate in efforts to improve patient flows and collaborate with other professionals to ensure the best possible care.
• Work closely with both patients and their relatives to create long-term and sustainable care relationships.
• Participate in quality work and development projects to improve the health center's methods and results.

Who are you?

• Licensed nurse or district nurse with at least two years of experience
• Experience in home care
• Thrives in a dynamic environment and can handle varying care situations
• Communicates clearly and has an empathetic approach
• Masters Swedish in speech and writing (at least C1 level)
