The Ystad Health Center is a well-functioning health center located in central Ystad, easily accessible by public transport as well as by car. We have a broad competence in health and medical care where we meet the patient's needs in several different areas. Furthermore, we actively work with preventive care to strengthen the healthy and encourage patients to take responsibility for their lifestyle and health.

About 70 employees work at the health center, all striving to provide the best care to our patients. We offer a complete primary care service where nurses, district nurses, doctors, physiotherapists, occupational therapists, nursing assistants, dietitians, social workers, and psychologists collaborate with the patient at the center. The health center also has several training positions in various professional categories. Many employees have worked here for several years, while we also have a number of newer employees. This mix is a good and creative combination of experience and innovation.

JOB RESPONSIBILITIES

We are looking for a new nursing colleague with a specialization in diabetes and a genuine interest in primary care.

As a diabetes nurse, you will meet various types of diabetes patients and work with, for example, adult patients with diabetes, newly diagnosed diabetics, and patients with prediabetes. You will also conduct annual check-ups and registrations in the National Diabetes Register (NDR).

We work to ensure the quality of care for our diabetes patients, which means that as a diabetes nurse, you will be engaged in following and improving quality indicators that have been established as common goals for the health center. Our goal is to focus on the patient's needs, where care is tailored individually and characterized by a professional approach and high medical quality.

In addition to your own outpatient work with diabetes patients, other duties at the health center will include telephone advice and triage.

Of course, as a new employee, you will receive a good introduction based on your existing skills and experience, including supervision for one hour a week with a designated supervisor at the health center. You will also be offered regular continuing education to continue developing in your professional role. In this position, you will have great opportunities to grow both professionally and personally.

We staff the evening and weekend reception at the Ystad Health Center together with several other health centers, so work in the evenings and weekends may occur.

QUALIFICATIONS

We welcome you who are a licensed nurse, or alternatively a district nurse, with the authority to practice within Swedish healthcare. Additionally, you have further education as a diabetes nurse. Language skills in Swedish corresponding to at least level C1 according to the Council of Europe's scale are required. If you have training in motivational interviewing (MI), this is an advantage. It is also advantageous if you have work experience from primary care and/or experience working in the PMO journal system.

The person we are looking for is confident and flexible in their professional role and comfortable working both independently and collaboratively with colleagues from various professional categories. You advocate for all our patients to receive a good response and professional care.
